Continence Nurse Consultant (Community) Support Registered Nurse

Wattletree Health Group, a leading provider of in-home registered nursing services in Melbourne’s North, Northeast, Eastern, outer Eastern and South Eastern metropolitan area and surrounds is looking for a continence nurse consultant (community) support RN with a background of continence nursing studies or strong passion for holistic Continence nursing passionate about delivering exceptional services and care in the home environment.

Client locations not limited to: Melbourne, Melbourne North, Northeast, East, Outer-East, South East.

Nursing environment: – On the road mobile service delivery to Home Care & Community, use of office for report writing/administration planning.

Casual: Wednesday, Thursday &/or Friday preferred, but open to negotiation (with an option to attend emergency call outs on weekends and after hours).

In this role, you will work under the direct management of our Continence Nurse Consultant and the WHG model of care and unique (and published) bowel management program.

Your role will include, but not limited to, catheter care and management, acute clinical symptom management and action steps, communicating with the office (daily debrief), bowel management implementation, comprehensive continence assessments, data gathering, background health information gathering and collaboration, report writing, NDIS plan writing, continence case management for highly complex clients, clear communication with external service provider case managers, product recommendation and ordering, among others.

Your communication skills, ability to deliver education modules in an engaging way, your knowledge, rationale-based decision-making skills and overall personality fit for this role is of particular importance.

You will be provided with extensive training and shadow shift support with assessments, catheter care, and continence troubleshooting across a diversity of client conditions in the community. You will exercise your critical thinking skills, troubleshooting skills, community nursing skills, and ability to make decisions in the moment in line with best practice and clinical support from our team. You will need to be proficient in report writing, documentation, sourcing of supplies, communicating with WHG clinical lead/case managers/physicians/specialists confidently and succinctly, in line with the WHG values and goals.

Your role will contribute to the continuity of care to our community and enjoy the collegial team culture we are accustomed to working with. There is always clinical support and admin support available 7 days a week, and after hours.

Start date: Feb 2024

KEY CRITERIA REQUIRED:

· AHPRA registration as Registered Nurse

· All mandatory police checks, manual handling, BLS, WWCC, current unrestricted drivers licence, Covid vaccination

· A minimum 7-10 years of nursing experience, with experience in home and community nursing preferred

· Excellent knowledge of bladder and bowel function and dysfunction, stoma care, wound care and all products relating to these aspects of care.

· Excellent ability to deliver education face to face.

· Agreement to deliver our WHG branded and IP protected bowel management program.

· Comprehensively insured car

· Smart phone

· Laptop/desktop computer

· Colour printer/scanner
