What is the Support At Home Program?
The Support At Home (SAH) program is the Australian Government’s aged care funding program providing financial support for older Australians to receive clinical care and support services at home. The Support At Home program replaced the Home Care Package (HCP) program from 1 November 2025.
If you have been assessed through My Aged Care and hold a Support At Home package, you are entitled to use your funding to access clinical nursing services from an approved provider like Wattletree Health Group.

How Support At Home Nursing Works
Getting started is straightforward:
- Nominate Wattletree Health Group as your preferred nursing provider when speaking with your Support At Home package manager or care coordinator
- We liaise directly with your package provider to confirm services and arrange billing
- Your registered nurse visits begin — we invoice your package provider directly
You will not pay for nursing visits out of pocket, provided they fall within your package budget. We also work with your care coordinator and GP to ensure your nursing care is well integrated with your broader care plan.
Who Can Access Support At Home Nursing?
Support At Home nursing is available to:
- Older Australians who hold a Support At Home package assessed through My Aged Care
- Clients previously on a Home Care Package (HCP) who have transitioned to Support At Home
- Clients whose package manager or care coordinator has identified a nursing need
If you are not yet on a Support At Home package and need nursing support urgently, we can also see you as a private client while your funding application is being processed — and transition you to funded care once your package is in place.

Yes — Support At Home participants have the right to choose their preferred providers. Simply tell your package manager or care coordinator that you would like to use Wattletree Health Group for your nursing services.
I’m still on a Home Care Package (HCP). Can I still use Wattletree Health Group?
Yes — Wattletree Health Group provides nursing services under both Home Care Packages and the new Support At Home program. From 1 November 2025, new packages are issued under Support At Home; existing HCP clients are being transitioned progressively. We can work with you under either funding arrangement.
What if my nursing needs exceed my package budget?
If your nursing needs exceed your Support At Home package budget, we can discuss a combination of funded and private nursing visits to ensure your care needs are fully met without interruption.
Do I need a GP referral to access Support At Home nursing?
A GP referral is not required to nominate us as your provider, though your GP may need to be involved for certain clinical services. Your package manager can help coordinate any referrals needed.
