No referral is required to become a Wattletree Health Group client. We accept self-referrals, and also referrals from family and friends, GPs, hospitals, My Aged Care Home Care Package (HCP) and Support At Home (SAH) providers, NDIS Support co-ordinators, allied health, and all other healthcare organisations. We are registered NDIS providers, and also approved providers for TAC and WorkSafe.
Can WHG provide clinical reports to support a My Aged Care reassessment?
Yes. If you need a higher package level or additional nursing hours, our nurses can provide a written clinical assessment documenting your health needs, the frequency of nursing required, and the clinical rationale. This report can be used to support a reassessment request through My Aged Care.
