When you or a loved one needs care after a hospital stay, Wattletree Health Group‘s experienced registered nurses provide professional, personalised support to help you recover safely at home — on your terms and in your own space. No waitlists. Available 24/7.
Why Choose Our After-Hospital Care Services?
- No waitlists. Hospital discharge can happen fast. We aim to have an experienced nurse with you the same day or next day — so there’s no gap between leaving hospital and starting your recovery at home.
- Comprehensive Care Plans. Our experienced nurses deliver tailored care plans covering wound management, medication administration, health monitoring, and more — designed around your discharge summary and your life.
- 24/7 Availability. Whether you need a few hours of support or around-the-clock care, our team is available day and night to ensure your safety and comfort at home.
- AHPRA-Registered Nurses — Not Support Workers. Every nurse who visits you is an experienced, AHPRA-registered Division 1 RN — not a support worker. You get genuine clinical-grade care at home.
- Faster Recovery at Home. Research consistently shows patients recover better in familiar surroundings. We help you get there safely.
- Reduced readmission risk. Consistent clinical monitoring at home catches complications early — reducing your risk of returning to hospital.
- No hospital-acquired infections. Recovering at home means avoiding exposure to hospital-acquired infections during your most vulnerable period.
Our After-Hospital Care Services
- Post-Surgical Care. Wound assessment and dressing changes, drain tube management, staple and suture removal, VAC dressing care, and pain management following surgery.
- Chronic Illness Management. Clinical nursing support for complex conditions including diabetes, heart failure, respiratory disease, and renal conditions — including medication administration, observations, and liaison with your treating team.
- Care for Older Adults. Specialised support for older adults transitioning home from hospital, including falls risk assessment, medication review, mobility monitoring, and coordination with GPs, specialists, and allied health.
- Medication Management. Safe, accurate administration of oral medications, injections, IV infusions, and syringe drivers — with regular review and GP liaison where needed.
- Health Monitoring. Regular observation of vital signs, wound healing, fluid balance, and overall recovery progress to catch complications early.
Funding Your After-Hospital Care
After-hospital nursing care may be covered by a range of funding pathways including TAC, WorkSafe, My Aged Care Home Care Packages, NDIS, and also privately (self funded). Wattletree Health Group is a registered NDIS provider and an approved provider for both TAC and WorkSafe. Contact us to discuss your situation and we can help identify the right pathway for you.
Frequently Asked Questions
How soon after hospital discharge can a nurse visit?
We aim to have an experienced nurse with you the same day or next day in most cases. There are no waitlists. If your discharge is planned in advance, we can arrange care to begin the moment you arrive home.
What’s the difference between a nurse and a support worker for post-hospital care?
A registered nurse is an AHPRA-registered clinician trained to assess, monitor, and manage clinical conditions — including wound care, medication administration, IV therapy, and recognising early signs of deterioration. A support worker assists with personal care and activities of daily living but is not qualified to perform clinical tasks. All Wattletree Health Group visits are conducted by experienced Division 1 registered nurses.
Does private health insurance cover home nursing after hospital?
Many private health insurance policies include a home nursing benefit for post-hospitalisation care. Coverage varies between funds and policies — we recommend contacting your insurer directly to confirm your entitlements. We can provide the clinical documentation your insurer may require.
Can after-hospital care be funded through My Aged Care or NDIS?
Yes. Post-hospital nursing visits can be funded through a My Aged Care Home Care Package (HCP) or Support at Home (SAH) program, and through NDIS for eligible participants. Wattletree Health Group is a registered NDIS provider. Contact us to discuss your funding situation.
Do I need a doctor’s referral to arrange after-hospital nursing care?
No doctor’s referral is required to become a Wattletree Health Group client. We accept self-referrals, and also referrals from family and friends, GPs, hospitals, My Aged Care Home Care Package (HCP) and Support At Home (SAH) providers, NDIS Support co-ordinators, allied health, and all other healthcare organisations. We are registered NDIS providers, and also approved providers for TAC and WorkSafe.
